Disability and Aged Care Services

Personal support (up to 24 hours)

Support or care is delivered by a helper for a minimum of 1 hour and up to 24 hours a day. Helpers will assist with various daily living tasks including:

  • personal hygiene
  • dressing
  • feeding
  • meal preparation
  • ambulatory assistance
  • medication supervision or prompts

Domestic support

Domestic support is ideal if you require assistance with day-to-day tasks such as light home maintenance;

  • gardening
  • cleaning
  • ironing
  • meal preparation
  • shopping
  • other light house chores

In-home Nursing Services

A dedicated nurse will provide short-term care and monitoring in your home. For example, a period following a hospital admission as you continue to recover or during times when hospital is not your preferred option and it is safe for you to receive care at home.

Our nursing team has extensive healthcare experience in working with people who require: Wound care, Urinary Catheter care, Tracheostomy care, Medication administration, Continence assessments and many more.

Overnight-Support

Overnight support

Support or care is delivered by a helper who will either sleepover or work an active night to monitor for safety and meet your support or care needs overnight.

Overnight support is ideal for those times when family or usual supports aren't available to assist. Overnight support can help with:

• Providing care and supervision overnight

• Giving respite to primary caregivers

Respite Care

Respite can be convenient when family or usual supports aren’t available or simple need a break. On such occasions you do not need to go to a residential facility or worry about being lonely. You will be able to receive personalised support whilst you remain in the comfort of your home.

We also know that hospital stay can be daunting. Therefore, we can easily get you support and care for you to receive certain treatment whilst at home instead of hospital.

Supported Independent Living- SIL (24/7)

You will be able to receive supports from your own choice of residence. Supports delivered under SIL include supervision with day-to-day tasks, ensuring your safety, developing and building your capacity in daily living and social skills.

The above could be achieved through a range of activities that we facilitate on day to day as part of our health and wellness program i.e art, cooking, exercise, social outings, and other various educational and creative activities.

 

 

Companionship and Community access

Living alone can be lonely and isolating. Community access will minimise or prevent the impact of loneliness and isolation, known to contribute to poor quality of life, stress and consequently poor mental and physical health. It will also help you learn new skills and improve your quality of life.

You will be supported to attend appointments and leisure activities

Our services are delivered on demand to suit your needs.

Funding & Payment

You can receive services through us:

  • If you are an NDIS participant with plan or self managed NDIS funds
  • If you are approved or in receipt of a Home Care Package (HCP).
  • By paying from personal funds outright using secure payment methods
  • In certain circumstances your Private Health Insurance might contribute to the cost of services. Check with your Private Health Insurance provider what they can cover or fund

If you currently don’t have either of the above funding, don’t stress. There is government funding available for older people (aged 65 and above) and for people with a disability. Government subsidy or other types of funding may cover all or part of the costs. If you require more hours than funded, you can top up using personal funds as and when required.

You can independently apply for your own funding. Alternatively, contact us or our partners to assist you to apply for home care funding, for free. Our partners Trilogy Care  can also manage your funding at an affordable price.

Home care package recipient:

Our partners can support with Home Care Package management at a low cost compared to other providers on the market. They can also assist you to self manage your Home Care Package, which allows you the freedom to choose and arrange support services and saves you money. The savings realised through engaging our partners will leave you with more funds available to pay for  day to day support services.

NDIS participants: 

If you are self managing your NDIS funding, you can book services via our platform at anytime.

If your NDIS package is plan managed, have a chat with your support coordinators and plan manager regarding engaging service providers on our platform. In the meantime, you can get in touch with us to discuss your needs and help options.
